How to Prepare for Your First Visit

This page helps new and returning patients prepare for their appointments, understand how care works, and feel more comfortable before their visit.
Complete any required forms: If forms were emailed or provided in the portal, fill them out before your appointment to save time.
Bring important information:
Current medications
Past mental-health or medical history
Any recent evaluations or reports
A short list of symptoms or concerns
Find a quiet, private space for telehealth: This ensures confidentiality and helps you feel more at ease during the discussion.
Arrive a few minutes early: For telehealth, log in 5–10 minutes beforehand. For in-person visits, arrive with enough time for check-in.
Medication safety is a shared responsibility.
Follow these simple guidelines:
Take medications exactly as prescribed.
Never change your dose without discussing it with your provider.
Store medications safely and out of reach of children.
Report side effects or unusual symptoms as soon as possible.
Avoid sharing medications with others.

Most follow-up visits last 20–30 minutes.
